Where is what I can only think of calling “quality control” at the editing and publishing levels?

Clearly, some of these authors are people with worthwhile ideas and the ability, more or less, to share them; but I just keep wondering: how does this stuff get into print without someone, somewhere along the line, putting on the brakes and ordering revisions in order to put out a higher-quality (that word again!), more polished product?

But, anyway, thanks for just the giggles I needed at this particular time, especially “…how green was my chakra” and …”mother’s mother”. Got good old-fashioned belly laughs out of each.
